CHANGE OF OWNERSHIP

TERMS & CONDTIONS

1. Applications are to be submitted by the owner(s) or his/her agent. If acting as agent, the application should be accompanied by a notarizedpower of attorney or written authorization in their favour from owner(s).
2. An application must be accompanied by proof of identification, i.e. PASSPORT (S), VOTER CARD (S) and/or DRIVER’S LICENSE (S)
3. In case of joint ownership, all owners e.g. husband and wife or their authorized agent must sign.
4. In case of Limited Company, the application must be signed by two or more directors of the Company and proof of incorporation.
5. In case of a church, not for profit or similar association, at least two trustees must sign.
6. A service deposit will be required. Cash, certified cheque or credit card payments accepted. Personal cheques are accepted fromcustomers with existing accounts in good credit standing.
7. I/We undertake to pay for all water registered by the meter at the rates and charges now fixed or which may hereafter be fixed by the Water& Sewerage Corporation or such minimum payments as may be fixed for supply and to conform to any regulations in force affecting suchsupply, or to any regulations hereafter made by the said Water and Sewerage Corporation.
8. I/We further agree, in the event of the fittings on my/our property not being in accordance with the regulations, to have all necessary
   repairs and alterations carried out by a licensed plumber to the satisfaction of the Water and Sewerage Corporation’s inspector and tomaintain such plumbing system in good order and condition.
9. It is distinctly understood and agreed upon that the Water and Sewerage Corporation neither is nor will be responsible for any defect,waste or misuse of water on the premises after it has passed through the meter, and I/we agree on behalf of the owner to accept fullLiability for all such quantity of water registered of tenancy of premises.
10. It is noted that upon failure to make payments on this account within six (6) months or more, the Corporation reserves the right to apply my service deposit to my outstanding balance, and refund any credits due to me resulting therefrom, should the assigned account befinalled and closed.
